You will have to sign and submit the reactivation form sent along with the email and submit any KYC documents if any information pertaining to your account has changed like your address, marital status, contact details and or bank details.

You will have to send this to the below address in Mumbai:

CHOICE HOUSE, Shree Shakambhari Corporate Park,

Plot No: -156-158, J.B. Nagar, Andheri (East),

Mumbai - 400 099.
